Who can use our Services
Services are available to people who work, live or study in the City of Whitehorse and we prioritise our services for people on low-medium incomes and holders of Health Care cards or Pension cards. Some of our services are provided for particular age groups, health conditions or disabilities, such as services to help frail older people and their carers continue managing independently in their homes; or services for young children (0-6 years).

| Community Health Nursing |
One-to one Community Health Nursing is available to people with chronic health conditions who need assessment and care to support them in managing their health issue and overall health. You need to work, live or study in the City of Whitehorse and we prioritise our services for people on low-medium incomes and holders of Health Care or Pension cards. To join the Life! program you must first be assessed by a community Health Nurse. |

Nutrition and dietetic services are available to people who work, live or study in the City of Whitehorse and we prioritise our services for people on low-medium incomes and holders of Health Care or Pension cards. |
| Occupational Therapy |
Occupational Therapy services are for frail older people, younger people with disabilities and others who need support to continue managing independently at home. You need to work, live or study in the City of Whitehorse and we prioritise our services for people on low-medium incomes and holders of Health Care or Pension cards. |
| Physiotherapy |
Physiotherapy Services are available to people who work, live or study in the City of Whitehorse and we prioritise our services for people on low-medium incomes and holders of Health Care or Pension cards. To join the following you must first be assessed by a physiotherapist:

| Podiatry |
Podiatry is for people who have a medical condition (such as diabetes or peripheral vascular disease) that effects their feet and may result in complications. Podiatry services are for frail older people, younger people with disabilities and others who need support to continue managing independently at home. You need to work, live or study in the City of Whitehorse and we prioritise our services for people on low-medium incomes and holders of Health Care or Pension cards. |

Children's Services

Dental Services are available to children who meet the following requirements regardless of where they live. Dental services are available and free of charge to all children and young people under 18 years of age who have a:
For those without the above concession cards, Dental services are available for all children from 18 months to 13 years of age with a one-off cost applied for a complete course of care. |

Diabetes Services

One-to-one diabetes nurse education is available to people with diabetes who need assessment and care to support them in managing their diabetes and overall health. You need to work, live or study in the City of Whitehorse and we prioritise our services for people on low-medium incomes and holders of Health Care or Pension cards |
| IDEAS – Integrated Diabetes Education and Assessment Service |
A ‘one stop shop’ to with a team to help you with your diabetes is available if you need specialist medical services to help you manage your diabetes and prevent complications. You must be referred by your doctor for this service. People on low-medium incomes and holders of Health Care or Pension cards are prioritised. |
Men's Health Services
