Heart Health Program

adults-exercising.jpgThe Heart Health Program is a comprehensive program that supports people in the management of cardiac conditions, including heart failure and other chronic medical conditions. As an integrated program it promotes:

  • Physical activity

To assist you with improvement in your physical and psychological well-being through a team approach

  • Health education

To provide an education program to both you and your family that helps you to understand your disease and its ongoing management.

  • Strategies to help modify behaviour

To provide a graduated exercise program that assists you to become physically active at a level compatible with the function of your heart.

  • Long-term support

To encourage and support you to become an active participant in the management of your disease and overall health and well being.

 

Can you use this service?

Referral to attend the program is required from your GP or Cardiologist contact the Heart Failure Case Manager at the Box Hill Hospital ph 9895 3333.
