IDEAS - Integrated Diabetes Education and Assessment Service

IDEAS = Integrated Diabetes Education & Assessment Service.

IDEAS is a program provided in collaboration with Eastern Health Endocrinology Department which provides comprehensive medical and allied health consultations including risk assessment, for people with Type 2 Diabetes. It aims to support people with diabetes to manage their general health and prevent diabetes complications.

We know that a team approach to management of diabetes resutls in the best health outcomes for people.  The IDEAS team includes:

- an Endocrinologist – a doctor specialized in diabetes care

- a credentialled diabetes nurse educator &/or community health nurse who will talk with you about your diabetes, your education needs & what else can assist you in your management of diabetes

- a podiatrist who will provide a foot assessment and advice regarding ongoing treatment needs & self care management.

 

IDEAS operates on a Monday afternoon at WCHS - Ground Floor Level, 43 Carrington Road, Box Hil

 

How do you make an appointment?

IDEAS is bulk billed, using your medicare card.  This means you need to have current referral from your general praticitioner and bring your medicare card to your appointment.

1.       Talk to your GP/doctor about attending IDEAS. They can arrange a referral via a fax to the Box Hill Hospital Endocrinology Out Patient Department on 03 9895 4852, marked ‘IDEAS’.

2.       Whitehorse Community Health Service staff will contact you to arrange your first appointment for IDEAS. Any further appointments will be arranged after your IDEAS consultation.

3. The IDEAS Endocrinologist will send a letter to your GP/doctor letting them know the outcome of your referral and if you have been referred to other services to assist your diabetes management.

Can you use this service?

IDEAS is available to people with Type 2 diabetes and a current Medicare Care card we prioritise our services for people on low-medium incomes and holders of Health Care or Pension cards.
