Diabetes Nurse Education

 

A diabetes nurse educator is available to assist if you are newly diagnosed with diabetes, or would like help with blood glucose monitoring, managing medications, or general advice about managing your condition.

Can you use this service?

One-to-one diabetes nurse education is available to people with diabetes who need assessment and care to support them in managing their diabetes and overall health.

You need to work, live or study in the City of Whitehorse and we prioritise our services for people on low-medium incomes and holders of Health Care or Pension cards
