Tell us what you think  ..

Whitehorse Community Health Service appreciates all feedback.  It helps us know how we are doing. Compliments are nice and we let our staff know the ways that you appreciate them and their work.  Any complaints are treated seriously. Often they help us improve the way we work or how we communicate with you.

Our promise  ..

We give you the choice of making anonymous or identified compliments and complaints. Your feedback can be confidential if you choose.  Regardless of how you give us feedback, we promise this will not affect your access to good treatment.

Four ways to give feedback

  1. Talk to staff or supervisor (at time of treatment or later by phone)

It’s best to initially give your feedback to staff or a supervisor.  Whether to show appreciation or express concern or disappointment, our staff are trained to be comfortable in receiving feedback (positive, negative or improvement ideas).  

We understand some people may not want to do this and also you may not get the response you would like.  So, in addition or instead, you can  ..

2.   Submit feedback via our Contact Us page

3.    Download Feedback Form and post through the mail to:

4. Complete Feedback Form at next visit and drop in Feedback Box
(forms and Boxes are on ground and 2nd floor, 43 Carrington Road, Box Hill)

What we do with your feedback

Compliments are passed on to staff – everyone likes compliments!

If a complaint is found to be due to a system or staff problem, our managers take action.  If you have requested it and give us your contact details, we get back to you with an outcome.

All feedback is recorded and reported (not your name) at management meetings.

 

The Victorian Health Services Commissioner

If you decide to take a complaint further, your next step is to contact the Office of the Victorian Health Services Commissioner on Telephone: (61 3) 8601 5200 or Toll Free: 1800 136 066.  The Health Services Commissioner receives and resolves complaints about health service providers with a view to improving the quality of health services for everybody.
